Sowing Seeds Health, Inc. proudly offers a School-Based Community Health Worker (CHW) Program designed
to address the physical, behavioral, and social health needs of students across 13 California counties.
This initiative integrates CHWs directly into public schools to provide accessible, equitable, and culturally informed
care navigation for students and their families—regardless of insurance status.
Counties Served: Sacramento, Long Beach (LA), Sutter, Yolo, Yuba, Orange, Napa, Humboldt, Shasta, Placer, Mendocino, Colusa, Glenn
🎯 Program Goals
- Improve access to primary care and behavioral health services
- Coordinate support for students with complex medical or developmental needs
- Promote preventive care, including immunizations and well-child visits
- Address housing, food, and transportation challenges
- Strengthen school-family-community partnerships
👨👩👧 Who We Serve
- Public school students and their families
- Medi-Cal-enrolled youth and those eligible for CalAIM ECM
- Children with IEPs, CCS-eligible diagnoses, or mental health concerns
- Uninsured or underinsured families needing help navigating the system
💼 What CHWs Do
- Partner with school health teams and participate in care meetings
- Screen for social determinants of health (SDOH)
- Help families access preventive health care and public programs
- Support CCS referrals and care plan follow-ups
- Provide on-campus support and direct outreach
- Connect families with resources for housing, food, and more
🚀 Implementation & Training
Phased Rollout:
- Planning: School district engagement, MOU execution, CHW training
- Launch: CHW deployment and referral workflow setup
- Delivery: Active screenings, referrals, and care coordination
- Evaluation: Data tracking, outcome reporting, and model refinement
CHW Training Includes: Trauma-informed care, motivational interviewing, FERPA/HIPAA compliance, youth behavioral health systems

💵 No-Cost, Accessible Support
All eligible CHW services are billed directly to Medi-Cal Managed Care Plans—there’s no cost to the schools or families.
Services are available to all, regardless of coverage or ability to pay.
